Steps to Apply for UP NHM CHO 2024
To apply for the Community Health Officer (CHO) position in Uttar Pradesh, candidates can follow these steps for the online application process:
- Candidates must apply through the NHM website in English. No other way to apply will be accepted.
- It’s important to fill out the online application correctly. Any mistakes are the candidate’s responsibility.
- Candidates should enter their Name, Father’s Name, and Date of Birth exactly as shown on their Matriculation certificate. If there’s been a name change, they should include the new name in the application. Other details must match the certificate. Any legal documents about the name change must be submitted during Document Verification.
- Candidates need a valid email ID and mobile number that should stay active throughout the recruitment process. Important information will be sent to this email, so candidates should check it regularly, including the spam/junk folder.

Eligibility Criteria for Community Health Officer (CHO) Positions
- Qualification: Candidates must have completed a B.Sc. (Nursing) with a Certificate in Community Health for Nurses (CCHN) or a Post Basic B.Sc. (Nursing) with the same certificate from a recognized institute or university from 2020 onward. This includes students who finished their 4th year of B.Sc. Nursing or 2nd year of Post B.Sc. Nursing in the year 2019-20 or later and passed the course.
- Nursing Registration: Candidates need to be registered with the UP Nurses & Midwives Council. Those registered in other states can also apply, but they must submit their UP registration certificate within 90 days after they join if they are selected.
- Affidavit Requirement: If candidates do not have UP registration but are registered in another state, they must submit an affidavit on a Non-Judicial stamp paper of Rs. 100 during the Document Verification Process (DVP). This affidavit states they will get their UP registration within 90 days of joining.
- Provisional Joining: Candidates who join based on registration from another state will be in a provisional position. Their role as CHO will continue only after they submit the UP Nurses & Midwives Council registration certificate.
- Candidature Cancellation: If a candidate does not provide the UP registration certificate within 90 days, their application will be canceled. The District Health Society (DHS) will issue a notice, and the candidate will have to pay the surety bond amount (Rs. 2.50 lakh) to the DHS.
- Current CHOs: Candidates who are currently working as CHOs in Uttar Pradesh cannot apply for these positions.
- Age Limits: Candidates must be between 21 and 40 years old as of the last date for online application submission.
- Age Verification: The date of birth recorded in the Matriculation/Secondary Examination Certificate or an equivalent certificate will be accepted for age determination. No requests to change this date will be considered.

UP NHM CHO 2024 Selection Process
The selection process consists of two phases:
- Phase-I: Computer Based Test (CBT): Candidates will take a Computer Based Test. Merit will be determined based on the scores from the CBT.
- Phase II: Document Verification Process (DVP): Candidates who are shortlisted based on their CBT scores will be invited for the Document Verification Process.
- Important Note: If a candidate is unsuccessful in the Document Verification Process, their candidature will be canceled.
Community Health Officer Exam Pattern
- Duration: The test will last for 2 hours in one sitting.
- Total Marks: The CBT will consist of 100 marks divided into two sections:
- Section-I: 80 Marks (Professional Knowledge related to the discipline/domain)
- Section II: 20 Marks (General Aptitude, Reasoning, General Awareness, and Basic Computer Knowledge)
- There will be 100 multiple-choice questions (MCQs).
- Each question carries 1 mark.
- There is no negative marking for incorrect answers.
- No marks will be awarded for questions not attempted.
- Language: The question paper will be available in bilingual format (English and Hindi). In case of any discrepancies in the Hindi version, the English version will be considered valid and final.

UP CHO 2024 Salary
- Salary: Successful candidates will be appointed as Community Health Officers on a contractual basis at sub-center level Ayushman Arogya Mandirs (AAMs). They will receive a monthly honorarium of Rs. 25,000 (Twenty-Five Thousand only) and may earn up to Rs. 10,000 (Ten Thousand only) per month as a Performance-Based Incentive (PBI) after being posted in their allocated district.
- Surety Bond: Candidates must submit a Surety Bond of Rs. 2.50 Lakh (Two Lakh and Fifty Thousand) on a Non-Judicial stamp paper of Rs. 100 at the time of joining. This bond commits them to serve for a minimum of 3 years at the Sub Health Centre level AAM in Uttar Pradesh.
