The candidates are advised to bring the certificates and documents in original along with one set of its self-attested photo copies for Document Verification as per the list mentioned below.

• Downloaded copy of Admission / Intimation letter / Admit Card for certificate verification
• Print out copy of Online Application Form submitted by candidate during submission of online application (legibly signed by the candidate)
• Two recent passport size colour photographs of the same which was uploaded during submission of Online
• Application form
• HSC/10th standard pass certificate in support of educational qualification and age
• Pass Certificate of Odia language, if applicable
• Updated and valid Caste Certificate (if applicable)
• Certificate / document in support of claiming to be Ex-Servicemen along with UNDERTAKING (Format at Annexure-B) (if applicable)
• NOC from the Appropriate Authority in case of Ex-Servicemen who are to retire within six months from the last date of submission of Online Application indicating there in the date of enrolment, expected date of discharge and year of service rendered in Defence Service. (if applicable)
• Sports ID Card claiming as Sports Person (issued by the Director of Sports, Sports and Youth Services Department, Government of Odisha as per Resolution No.24808/Gen., dated the 18th November 1985 of General Administration Department) (if applicable)
• Aadhar Card issued by the Govt. Authority, with candidates’ photograph therein.
• NOC in case of candidates working in Govt. Service (if applicable)
• Legal documents in support of having more than one spouse (if applicable)
